Introduction
Postpartum Hemorrhage refers to the loss of blood after the birth of a child. Its primary cause is weak contractions of the uterus that result in poor blood vessel compression resulting in acute blood loss. It gets associated with low blood volume as a significant symptom. The purpose of managing postpartum hemorrhage is slowing and preventing excessive blood loss although Medical practitioners prefer and recommend the Four Ts in assessing Postpartum Hemorrhage.
Four Ts to Assess Postpartum Hemorrhage
Tone, this involves assessing the uterine muscle's loss of tone .Uterine atony requires an assessment of the failure of the uterus to contract, resulting in acute bleeding (Anderson, 2007). After the child's delivery, the uterus contracts and pushes the placenta out. If the Uterine contractions do not pressurize and compress the bleeding vessel, they will bleed freely. Chances of coagulation will be low, resulting in acute bleeding.
Trauma involves assessing for any injuries that may have occurred to the birth canal during delivery. It consists of evaluating the perineum, vagina, uterus, and cervix (Sanders, 2019). Bleeding is severe during and after birth since pregnancy curtails all these organs getting vascular.
Tissue retention. During childbirth, tissues from the fetus or placenta may get retained. These tissues are later on passed out and as blood clots. Abnormalities in the placenta and passing out the retained membranes that may result in severe bleeding.
Thrombin refers to a disorder that impairs blood coagulation and prevents blood from clotting (Sanders, 2019). Childbirth gets characterized by ordinary and durational hemorrhage. Mothers with thrombin or coagulopathies may experience severe blood loss since, even after the normal blood loss during childbirth, their body is incapable of coagulation, thus cannot stop the bleeding.
Amy is experiencing postpartum anxiety. It can get diagnosed from her explanation when crying which indicates she is worried and how she says that since she delivered she suddenly feels overwhelmed and anxious. I would counsel and assure her that it is normal for some new mothers to experiences anxiety disorders resulting from sleep deprivation, getting overwhelmed with the new responsibilities of taking care of the child and severe hormonal changes. I would motivate, encourage and tell Amy that the anxiety disorder is manageable and treatable since it is not severe and se sought help early enough. I would offer her the following remedies psychotherapy that would involve talking and engaging Amy in conversations with a counselor to maintain proper mental health (Targonskaya, 2018). Cognitive behavior therapy that would involve Amy getting taught how to identify conditions, behaviours or thought which trigger her anxiety and change them. I would advise Amy on some self-care tips that curb postpartum anxiety such as maintaining a healthy diet intake and avoid drinks containing high caffeine contents (Targonskaya, 2018), regular exercising, maintaining regular and healthy sleeping patterns and engaging in relaxing techniques like practicing yoga which help stabilize moods.
