What Are The Differential Diagnoses?

A clinical criterion for the diagnosis requires that nurses and clinicians take a patient through an evaluation of their tics, as well as the patient's mood and anxiety disorder. The nurses should also consider compulsive behaviors like neurodermatitis and trichotillomania. The patient may also be diagnosed for anorexia nervosa and occasional schizophrenia.

What Are the Patient's Diagnoses? Comorbid Diagnoses If Any.

From the diagnosis, the patient has obsessive compulsive disorder (OCD). From the patient's history, he has developed a fear for HIV, alongside anxiety, depression, and concerns about appearance. The patient also requires a comorbid diagnosis since he has shown symptoms irrelevant to OCD like an urge to harm self and others.

What Level of Care is Appropriate for the Patient?

The OCD patient should always ensure that he follows the prescriptions of his medications since if he fails to follow it, the symptoms may return. He should also pay attention to signs that warn about their disorder manifesting. Also, he should consider consulting his doctor should he wish to change medications.

What Screening Tools Would You Use if Any?

In addition to Lewis' history, I would confirm if he keeps checking things over and over again. I would also inquire if he makes any attempts to set himself free of the thoughts that come to his mind. Moreover, I would find out if the condition troubles him.

What Medications Would You Prescribe To Include Dose And Schedule? Side Effects? Interactions?

I would prescribe paroxetine, a medication that is used on adult OCD patients. He would start with 20mg per day, and increase the dosage by 10mg per week. However, he should not consume above 60mg per day. During his medication, he should expect nausea and insomnia. He may also experience headache, constipation, and dry mouth.

What Diagnostic Labs Would You Order?

Unfortunately, there is no lab test for OCD. Medical practitioners solely depend on symptoms, as well as the results of the overall functioning of the patient.

What Are the Red Flags in This Patient's Presentation?

Despite first medication, the patient's symptoms showed again. Besides, even after being enlightened in school, he still believed that he could contract HIV by other body fluids like sweat.

What Would Other Treatments In Addition To Medications You Implement?

Other than medication, I would implement cognitive behavioral therapy (CBT) and talk therapy to make the patient understand better his disorder.

Any Additional Information You Would Like To Request, If Any.

I would wish to know if any other members of the family of the patient show similar symptoms as Lewis so that they too get treatment, at an appropriate time.
