Alaska State Board of Pharmacy
Reciprocity applicants must provide official verification of licensure (VOL) from each state in which one has ever held a license as well as the expiry of the permit. Completed VOLs need to be mailed directly from the former state board to Alaska State Board of Pharmacy. Original documents should be used. Certificates of Moral Character and the Verification of Work Experience should be notarized. Foreign graduates must provide a certified diploma copy from an accredited university.
Applicants for pharmacy technician roles should be able to speak, read, and write in English. Additionally, they must possess a high school diploma or GED. A pharmacy technician pays an initial application fee of $120 that covers the non-refundable application fee of $60 and an initial technical license fee of $60 (Alaska Pharmacy Technical). Notably, the Alaska State Board of Pharmacy offers temporary pharmacist licenses. The only document not required for temporal licensing is NAPB final application for license transfer.
Iowa State Board of Pharmacy
Pharmacy applicants have to undertake the North American Pharmacist Licensure Examination (NAPLEX) and Multistate Pharmacy Jurisprudence Examination (MPJE). Foreign Pharmacy Graduate Examination Committee (FPGEE) shall give certification for foreign pharmacy graduates.
College graduates provide a certificate from a college of pharmacy with either a Bachelor of Science degree in pharmacy or a doctor of pharmacy degree. Applicants cannot obtain certification for PTCB or NHA without a diploma or GED. A biennial license fee including a surcharge fee is levied. More specifically, a $72 processing fee not liable to refund is a requirement. Temporary permits are available with a provision of licensee supervision.
Nevada State Board of Pharmacy
Pharmacy technicians should finish a training program accredited by ASHP. Both the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE) certification are required. No restrictions for pharmacists reciprocating from Florida. Applicants for pharmaceutical positions must have no history of drug abuse (How to Become a Pharmacy Tech).
Graduate Certification for a pharmacy technician is not necessary as a number of options including ASHP accredited training program, federal armed service training, and 1500 hours of training documented by an employer could serve in lieu of accredited graduate certification. Application for Pharmacy technician demands a fee of $40 along with a completed Pharmaceutical Technical Application form.
Oregon State Board of Pharmacy
Technicians qualify after passing the national pharmacy technician certification exam. Reciprocity and endorsement applicants must have been practicing pharmacy for at least one year including a minimum of 1440 hours of work experience as a licensed pharmacist. NHA require applicants to provide proof of training from accredited institutions.
Applicants must be 18 years and above with a high school diploma or GED. Passing a criminal background check is essential for qualification. Both NAPLEX and MPJE certification is required. The initial cost of Pharmacy technician certification is $129, and it is renewed after every two years at the fee of $40. Markedly, the Oregon State Board of Pharmacy offers temporary licenses.
Arizona State Board of Pharmacy
Applicants must have both North American Pharmacist Licensure Examination (NAPLEX) and Multistate Pharmacy Jurisprudence Examination (MPJE) certification. Reciprocity applicants licensed in California are required to contact the Arizona State Board of Pharmacy. Applicants seeking pharmacy technician licensing must have a high school diploma or an equivalent and be at least 18 years of age. Pharmacy technicians pay an initial licensing fee of up to $82.00.
