The health indicators in this scenario are sexual orientation, restricted education, low income, and race of the people. Also, the population targeted in the case is of people who cannot access healthcare, and people whose health conditions and lifestyle may restrict their length and quality of a healthy life.

Other disciplines that I might work with are some people with chronic diseases in the area, people with disabilities in the area, people suffering from acute malnutrition, and musculoskeletal disability, health insurance, a safe healing environment that will not worsen the illness, and easy access to healthcare centers. It is important to work from a team perspective because you will be able to gather ideas, review them, and chose the most suitable one for the best interest of the patient. As a team, work will be easy to manage since everybody will be tackling his / her chores, and this will enable you to achieve your goals faster (Allender, Rector, Rector, & Warner, 2013). When you work as a team, you can solve your problems and conflict easily in work and also, you will be able to build trust among each other. When you work as a team, there is the permission of brainstorming, which allows the workers to come up with new ideas that will help in solving the problems easily.

Before Muharem is discharged from the hospital, we will take his details and his home address. Also, we will take the home address and the contacts for his friend, so that we can speak to him when Muharem is not around. Muharem's friend, who speaks a little English, should stay with Muharem, so that in case of any emergency needed, and he will call the hospital. Visiting days will be placed twice a week, to check on how muharem is doing, and also so that we can provide him with the medical supplies. We will examine the health indicators keenly so that we can determine what is best for muharem, and if he is receiving the right treatment. Muharem will be allowed to come back at the hospital once in a while for a health checkup, and if the environment he is living in is best for his recovery.
