Introduction
The Medinet Family care clinic's smoking cessation for the elderly is an initiative enhanced in the community to empower and motivate old adults that have the disadvantage to quit smoking (Bachman, 2012). The role of the clinic is to offer a fresh start and outline programs that involve their patients in sessions that last up to four weeks, assist in tracking people that use tobacco to quit smoking and help them get an effective quit plan.
Project's Objectives and Aim
The Aim
The primary aim of this project is to create an integrated approach related to smoking by offering the patients long-term support not only to reduce or quit their use of tobacco but also to assist them access and recognize a consistent long-term health improvement plan.
Objective
To highlight various difficulties such as the lack of access to local smoking cessation services mainly for the old that live in environments of social-economic disadvantage.
The Objective of this project is also to issue an integrated approach to smoking cessation not only to support but also to advise the patients before, during and after quitting aiming to change their lives (Bilgic & Gunay, 2018).
To improve the independence of old people including their well being based on the promotion of health.
To provide recognized and accessible link for the old to ensure they get access to broader health improvements options and opportunities.
Objectives and Goals Under Healthy People 2020
The department of human and health services which was launched in Texas Houston had four overarching goals;
Ensure that high-quality services are offered to give patients longer lives that are free of disabilities, preventable diseases, premature death and injuries (Tiona, 2010).
To develop social and physical environments that facilitates healthy living for every citizen
To offer healthy behaviour, healthy development, and good life across to all citizens
Healthy People 2020 has 1,200 objectives set into 42 different areas to represent an excellent public health area. By 2010, the organization placed its goals measured with established targets and baseline data. Healthy people 2020 also have a new foundation section that deals with vital health topics.
Background
The Medinet Family is a care clinic that opened its doors in 1997 to offer residents with exceptional health services and healthcare. To provide excellent services to their clients, the organization established a walk-in and walked out rules to avoid booking of appointments with doctors. For the walk-in patients, the clinic works hard to ensure that each patient manages to see a physician (Bachman, 2012). The work-in-patients, on the other hand, have a flexible day and schedule that makes it convenient for them to visit the clinic. Lastly, the clinic makes sure that every patient that walks in their facility is treated with professionalism and kindness.
The Medinet Family medical treatment facility offers the latest technology to take care of their patients through highly experienced and trained medical staff and physician. One challenge the medical care treatment facility started dealing with is the subpar care, long wait times and limited services because it hindered the quality of service offered to their patients (Bachman, 2012). The medical facility opens its doors to everyone including the families that seek to obtain special medical care.
The Beneficiaries of the Project
The project will offer a tailored service to the hardened smokers as well as the elderly patients that are unlikely to join the smoking cessation services. The focus of the project is to offer intensive support to the old patients by providing them the chance to develop the confidence and skills which changes their lives by increasing their likelihood possible health changes made and sustained within their communities (Bauld, 2010). The groups that managed to quit smoking are still assisted in helping reduce the risk of tobacco and ill-health related diseases by raising awareness to the community through the knowledge that associates with smoking and the smoking cessation services.
Services and Time Frame
The project will run from April 2019 to March 2020 by providing a free smoking cessation service during the day at local places. The project will also offer tie services at one-off events to ensure that they raise the awareness of good health. The primary venues targeted for this initiative include local rent offices, adult learning centers, and community centers. The local citizens that will attend the smoking cessation services will also be done through self-referral including the Medinet Family care clinic past work to cover everyone.
The Teaching Material Used To Execute the Project
The project will initiate a one-hour training session before the resident physicians and residents are hired. The courses at the training will begin with the introduction of the program, procedures, and purpose as well as educational materials that give guidelines to smoking cessation services from Healthy People 2020 (Tiona, 2010). The protocol of the program will require the available physicians to implement the 'first four' models for the patients. A fifth model which will also be initiated which will serve as an intervention protocol. The patients will then be questioned concerning their smoking status, the accessibility of nicotine substances and then advised by the physician on how to quit smoking with the assistance of self-help pamphlets, nicotine patches or by the referral to the Healthy people 2020 organization.
The Management and Funding
The project will be sponsored a significant amount by the Healthy people 2020. Since the Medinet Family care clinic will manage this project, the organization will also provide invaluable access to resources and training support to the appointed smoking cessation co-coordinators who will act as advisors during the early planning of the project.
External Action Plan
The American Lung Association is a firm that gives aid for local and state programs which deal in assisting individuals that smoke to quit so that the children may not find their selves into the same situation. Houston Texas, the District of Columbia and 50 other states have tobacco quite and prevention programs. In most cases, the Association is funded by the tobacco settlement dollars. The organization has unmatched flexibility programs that offer self-help support, and online phone communication that tailors a program that fulfills the client's unique needs. They also have a systematic approach to quitting that help the patient's to design an action plan that can make them quit day smoking. The association uses a routine behavior based method depending on the current behavior and addiction change models as well as the challenges the patient may face while quitting smoking.
Besides taking care of the patients, the association has reasonable standards like the affordable care Act that assists the health insurance and employers to meet the requirements when it comes to tobacco cessation treatment plan. The program also conforms the counseling of the smoking cessation as well as the core measures of the accredited body in health care. The program also contains the intensive intervention, created and released by the U.S health and human services to ensure high-quality services are given to the patient (Purdy, 2010). The timeline under these policies is one year, and the below will be the expected outcome once the project commences.
The project will offer skills to the patients to assist them to change their lifestyle choices and long-term health instead of struggling to quit smoking. The smoking cessation, in this case, will be issued in four segments. The first segment will operate from April to June 2019; the second segment will be operational from July to September the same year. Section three will be from October to December the same year, and the last part will operate from January 2020 to March 2020.
The expected outcome is that 80 percent of tobacco users will quit smoking and never engage themselves in smoking again. Awareness will be created, and this will help reduce the number of smokers as most of them will be knowledgeable to understand the repercussion of tobacco. The expected outcome is that kids will not engage themselves in smoking because they would realize the impact and dangers of cigarette. When the rate of smoking decreases, then the chances of having meeting individual smoking is minimal. This way the chronic diseases associated with smoking will also reduce, and this makes an individual live a healthy life.
