The Purpose of the Program
The primary purpose of this program is to improve patient safety and ensure evidence-based clinical interventions on resident patients. Patient safety plays a significant role in overall patient satisfaction and also reduction of possible readmission. The program aims at improving healthcare provider's communication between service rounds to ensure all the patient's needs are accounted for during every health care providers visit. The program seeks to ensure that communication channels and procedures in the healthcare center are more effective and patient-centered to ensure that the healthcare providers are aware of every patient needs. From an organization point of view, patient management can only be fast-tracked through improved internal communication to ensure that the patient information within an organization is easily accessible and the healthcare providers are able to share ideas and solutions for clinical interventions for the resident patients (Shojania et al., 2001, p. 668).
In most cases, patient management does not fail due to the lack internal patient management programs and procedures but the lack of proper communication channels. The communication gaps between health care providers, especially during the rounds and different patient attendance by different healthcare providers, reduce safety due to different perceptions caused by lack of a common concept of the patient. Patient safety can be assured and greatly improved through the use of the better communication initiative between the healthcare providers. Most of the resident patients have serious problem and healthcare provider's mistakes that are caused by lack of proper information sharing poses a significant threat to patient safety and the program will ensure that resident patient's information is availed to all attending healthcare providers to improve patient progress assessment and improve overall care (Firth-Cozens, 2001, p. 31).
The Target Population or Audience
The primary target population for this patient information management program is the health care providers who are in charge of the resident patients. The healthcare providers taking care of the resident patients especially those that have chronic and serious conditions are nurses and physicians who have different shifts. Improving information sharing between healthcare providers within a healthcare organization will improve patient care decisions and interventions to ensure that they are evidence-based and more patient-centered. The target population will benefit from easy patient attendance and information sharing which reduces common healthcare provider's mistakes that hinder patient safety.

The patient management through better communication program will have significant benefits to the healthcare providers, patients and the healthcare centers that are responsible for providing necessary care resources.

The primary benefit to the patients is the improved patient safety and reception of the evidence-based and patient-centered care. The program will reduce the overall resident patients stay in hospitals as a result of evidence-based care and better patient decision making. The cost of healthcare will significantly reduce through improved communication between healthcare providers which improves patient management and reduce hospital days. In addition, the program will help calm the family of the patient by information sharing patient information and goals through the patient daily progress note that is attached to the patient information file. Patient safety plays a significant role in overall patient wellbeing and the primary role of the program is to improve patient safety which can reduce possible morbidity that is caused by healthcare provider's errors especially to the patients suffering from chronic diseases such as cancer and heart problems. The patient goals established by the program ensures continuous improvement of the patient and continuous monitoring which ensures that every intervention and decision by the healthcare providers makes a holistic assessment of the overall patient wellbeing (Firth-Cozens, 2001, p. 31).
Benefits to Healthcare Providers
The healthcare providers will benefit from the program because the program provides a framework for the healthcare providers to share information and expertise that can boost patient safety measures. Decision making is a major factor and source of the contest in healthcare regarding patient safety because health care provider's errors are caused by lack of patient information at the point of care. In the absence of the program, patient information is not readily available and decision making is highly hampered by this factor. The patient management program through better information sharing is a patient-centered approach that ensures that all the healthcare providers that are in contact with the patients have access to patient information and updates. In other cases, healthcare providers such as nurses have to multitask and provide care to multiple patients and the lack of patient information hampers evidence-based decision making and the quality of care. The program will ensure that nurses and physicians can be able to make evidence-based and patient-centered decisions (Lee et al., 2010, p. 417).
Benefits to the Healthcare Center
The sustainability of the healthcare center processes and healthcare services depend on the level of patient satisfaction. In this case, the healthcare center will benefit from the program due to the high patient satisfaction which will improve patient preference to have the healthcare center provide them with their healthcare needs. Most of the healthcare centers are profit based and it is only through proper patient management through better communication approach that the healthcare center can get a good reputation amongst the healthcare consumers who are today aware of the importance of evidence-based care initiatives (Durham & Alden, 2008, p. 20).
The Cost or Budget Justification
The program will cost approximately 10,000 dollars and the cost will go towards creating awareness through healthcare provider's education and familiarization with the approach. Another cost will go towards purchasing the communication tools that will be used by the healthcare providers to share information. Lastly, the implementation of the program will also incur the accumulative cost because it will need an oversight body that ensures that the healthcare providers observes all the program protocols.

The evaluation of the program will be important in ensuring that the program meets the healthcare needs of the resident patients as well as the objectives of the health center. In addition, the program evaluation will ensure that the resource of the health center is used appropriately.
