At the time of her admission, the appropriate nursing interventions to Mrs. Jose given her history of sleep apnea, chronic heart failure, and hypertension are those strategies that will help lower her blood pressure and hence reduce her heart problems. According to Araujo, Nobrega, and Garcia (2013), such nursing interventions include lifestyle modification, blood pressure monitoring, bed rest, administration of anti-hypertensive therapy, observation of vital signs, assessment of the history of hypertension, and monitoring of symptoms and complications.

The rationale for the administration of IV furosemide (Lasix) medication is to treat the patient's high blood pressure by removing excess water and sat from the blood. The purpose of administering Enalapril (Vasotec) to Mrs. Jose is to act as an antihypertensive agent that helps to lower a patient's blood pressure. For patients with congestive heart failure, Vasotec also helps decrease the frequency of hospitalization, increase survival, and improve symptoms. Further, Metoprolol (Lopressor) is administered to patients with Jose's conditions to help prevent heart attack or to treat hypertension and chest pains. The purpose of administering IV morphine sulphate (Morphine) is to help with severe pain treatment and management.

Some of the cardiovascular conditions that may lead to heart failure include Coronary artery disease, heart arrhythmias, myocarditis, and hypertension. Coronary heart disease is a leading cause of heart failure which occurs due to a buildup of fats in the arteries thus reducing the blood flow to the heart and other vital organs. What can be done to prevent heart failure is a change in lifestyles such as avoidance of fatty foods and exercising. Heart arrhythmias or abnormal heart rhythms is a condition whereby the heart beats too fast thus getting overworked and hence leading to heart failure. To prevent this, patients should be placed under bed rest and avoid activities that cause the heart to beat faster than normal. Myocarditis refers to the heart muscle inflammation caused by viral infections. The appropriate intervention is to apply the relevant medical therapy while hypertension or high blood pressure is the condition whereby the heart is forced to work harder than normal due to weak or stiff heart muscles. What can be done is to use appropriate medical therapy.

Examples of nursing interventions that can help prevent problems caused by multiple drug interactions in older patients (polypharmacy) include furthering understanding and knowledge of medication use, facilitating independent medication management, and reducing pill burden (Farrell, Shamji, Manahan, & Merkley, 2013). Improving patient knowledge of how to use medication can help prevent polypharmacy by making them more informed of the adverse effects on their health. Also, proper medication management by pharmacies and hospitals can also help prevent polypharmacy by ensuring that medication is only issued to elderly patients based strictly on medical prescription. Moreover, reducing the pill burden can be helpful in that it will minimize the number of pills that elderly patients have access to. Additionally, behavioral modification strategies such as aromatherapy, pets, music, and heart as part of health care plan for the elderly can help prevent polypharmacy in that they help reduce the need for frequent medication.
