To effectively ensure that I was able to assist the patient, I was able to set a priority to patients who are of Hispanic background so that I would be able to understand more about their culture through active interaction (Bernal, 2009). For this intervention procedure, I was mainly inclined towards dealing with elderly Latinos living in a hospice facility. Although hospice care is considered to be a gold standard for issuing health services for individuals with terminal illnesses, research studies indicate that there is decreased representation of patients from ethnic and racial minorities in the country. Among those affected by this irregularity are people from the Latino community. As such, the need exists for coming up with relevant measures aimed at ensuring that all individuals are adequately represented in as far as the provision of medical healthcare is concerned.

The reason behind this was based on the fact that this minority community has been denied adequate access to hospice services over the years as a result of underrepresentation. Also, a significant number of elderly Latino people belong to the low-income population margin hence making it particularly difficult for such patients to obtain relevant care.
Describe networking experiences and how you are or will navigate community organizations designed to serve Hispanic children and families.
As an act of gratitude, I was able to discuss the matter with a local hospital in my neighbourhood challenging the fact that was high time that the medical industry took the initiative of ensuring that all individuals are equally represented. This was essential as I was given an opportunity enrolling elderly Hispanic patients termed to have terminal illnesses. As part of my profession, I was then mandated with the duty of taking care of the new arrivals at the hospice centre.
Describe a case where you collaborated with a Hispanic client and how you incorporated culturally significant familial or community relationships in the intervention process. Include use of mutually agreed on outcomes, and how you evaluated effectiveness of the intervention.
Some of the daily care routine that I was regularly involved in towards helping the patients included ensuring that they are availed with relevant food products and can obtain their daily dosage of medicine about individual illnesses.

In addition, the patients were also occasionally encouraged to engage in social activities amongst each other. This allowed all patients to effectively interact with each other hence reducing the burden associated with the stresses of knowing that they are likely to die at any minute.
How did you apply interventions that are culturally responsive, client centered and evidence-based?
As part of the Hispanic culture, family is a very crucial part of each person's growth. As such, I made sure that introduced the family entity into the healing process. Similarly, in order to enable the client to enjoy a peaceful stay at the hospice center, I was also able to arrange family visits twice every week so as to enable the patient to partake a normal lifestyle he same way they were before their illness. By introducing family ties, the patient is more likely to introduce the factor of joy and love for the patient in question. In order to enhance the wellbeing of the patient, I was able to come into a mutual agreement with close family members on the visiting schedule where the family was allowed to engage in fun activities with the patient every Wednesday.
What are the importance of collaboration with a Hispanic client in the intervention process?
In the first phase of the intervention process, I was able to exclude one victim from the rest. This move was particularly essential as it allowed myself to effectively interact with the victim in an environment where he is able to obtain sufficient healthcare. In addition, this interaction enabled me to fulfill the primary goal which it to enhance the representation of minority community members in hospice care.
What is the importance of incorporating culturally significant familial or community relationships in the intervention process?
In the Hispanic community, individuals often tend to keep very close ties with their families and friends (Chartier, 2010). As such, I was able to apply a culturally responsive intervention measure which includes barring temporal communication between the victim and family members and friends. Also, I regularly engaged the victims in daily sessions that required him to learn the health dangers associated to his elderly condition. Similarly, the victim was also introduced to a healthy eating diet to enable his body to adjust to his physical health effectively (Bal, 2016). As such, this exercise was of great importance to my profession as it enabled me to teach more about the Hispanic cultural heritage through how the family members interacted with each other.
Importance in evaluating the effectiveness of the intervention
After a series of 3 consecutive months, the now-recovering victim was allowed to have supervised visits with strict members of his family. This was to enable the patient to understand the importance of family, and as such, it would be essential for him to ensure he lives a healthy life if he is to see them on regular basis (Bernal, 2009). This exercise was conducted for six months before it was made to be a permanent routine. The need for collaborating with a Hispanic client was customarily essential as it served as the right step towards combatting the aspect of the underrepresentation of the Hispanic people as part of the racial minorities in the United States.
