Introduction

The Ottawa Model of Research Use (OMRU) is a comprehensive interdisciplinary framework of elements that affect the process of getting research findings that affect the process of applying research into practice. There are three domains, including assessment of barriers and support, monitoring interventions and use, together with the evaluation of outcomes. Six elements that make up the OMRU model are evidence-based innovation, potential adopter, practice environment, interventions that promote adoption for short-term and sustained use, assessment of outcomes.

The OMRU model is based on three assumptions which include the fact that it is dynamic, and all elements interact with and influence each other. The other assumption is that patients together with clients withhold a significant responsibility in all the dimensions of the model. Similarly, society is also regarded as the external environment of the healthcare which influences all aspects of the process.

The OMRU model will be used in this project to inform an agent of change on to identify factors which entail either discouraging or supporting innovation adoption, potential adopters and to understand the culture of the environment of practice. This project aims to describe current safe guidelines while overcoming potential barriers and encourage facilitators of adult family homes regarding best practices in PRN medications management. The Ottawa Model of Research Use (OMRU) is presented in Appendix A.

Setting and Population

The setting of the present project will be the Adult Family Home Council (AFHC), located in Olympia, Washington. The AFH Council is an organization established to support AFHs in Washington State by providing training and resources. The AFHC works closely with DSHS, Department of Health (DOH) and other government agencies such as the Washington State Department of Labor and Industries to stay up-to-date on current information (J. Ficker J & K. Cordero, oral communication, November 10, 2018). The AFHC provides input on policy changes and any other issues pertinent to the AFH industry. The AFH providers are directly responsible for the safe handling of PRN medication administration for their residents and use the AFHC for consultation. This project intends to serve individuals who use the services of Adult Family Home Council.

Objectives

Analyze the content of citations to understand common issues identified by the DSHS field inspectors.

Identify facilitators and barriers that affect implementing safe PRN medication administration practice in AFHs from the perspectives of AFH providers as well as nurse delegators and pharmacists who work with AFH providers.

Develop recommendations for the modification of the present materials provided by the AFHC using the data collected from the first and second Objectives and the relevant evidence from the literature. The purpose is to have resources that AFHC could share with AFH providers to help with their PRN medication management, thereby increasing the safe delivery of PRN medications and a decrease in the frequency of medication errors/citations.

Deliverables

An executive summary of the project findings and recommendations.

Design and Implementation Plan

The Ottawa model will be utilized to plan and design the project. For the communication plan, a weekly phone conversation will be held with the project contacts for the entire project implementation phase. Clarification and feedback on project activities will be solicited from the AFHC via email correspondence at least twice a week in order to achieve the goal of the project. The progress of project activities and progress towards completion of objectives and deliverables will be presented. At the same time, I will hold meetings with AFHC as needed in-person at least once a month to discuss the project activities. A final written report of compiled results, conclusions and recommendations will be included in the executive summary and provided to the AFHC by March 5, 2019. A poster presentation will be delivered to the University of Washington Doctor of Nursing Practice Supervisory Committee on March 15, 2019.

Objective 1.

The use of the Ottawa model will help with the creation of a reliable platform by which the DSHS field inspector will determine the reliable mechanisms of managing and controlling the medical errors that may arise in the FHS. The through this model, there will be attempts to control the attitudes of the DHS inspectors while also seeking to improve their knowledge in data information collection and analysis at large. Effective implementation of the Ottawa model will help the research to identify and to implement effective ways by which the investigation can identify barriers and to effective PRN medication while implementing strong and reliable solutions. This can be achieved through the use of the identification of the environmental adopters such as cultural and social background of the patients.

The adoption process with automatically help to ascertain the use of the findings in this field research by the DHS in the realization and creation of the effective recommendation that will see the establishment of a lasting solution to the FSH families and the final control of the healthcare challenges to the affected persons by limiting errors and managing the PRN medical condition to the patients.

Objective 2.

An easy-to-use questionnaire will be created to collect information from AFHs about PRN medication management. The AFHC and the School of Nursing faculty will assist in the creation of the questionnaire, and it will be completed by January 2019 (Potential survey questionnaire draft presented at Appendix B). I will work with AFHC contacts and ask them to assist with distributing the anonymous questionnaires to AFHs providers. These questionnaires will help gather information on barriers and facilitators that AFHs providers face while managing PRN medications in their facilities. Interviews of three nurse delegators and two pharmacists who work with AFH providers will be conducted in order to gather their concerns regarding PRN medication safety. An analysis of this data will be performed to identify facilitators and barriers that are mentioned by questionnaire respondents and interviewees by February, 2019. By the end of this process, the identification of the facilitators and the barriers that affect the implementation of safe PRN medication administration in AFHs will have been completed, accomplishing the second objective of the project plan.

Objective 3. You are still not clear about the use of model and the relationships between your objectives.

Following the accomplishment of objectives one and two, a thorough literature review will be completed, focusing on barriers and facilitators to the safe management of PRN medications experienced by AFH providers. Your objective 3 are about integration of the literature and results from Objectives 1 and 2 and use it to modify their current materials, if any, or create one if needed. Identification and review of the current PRN medication form that AFHs are using will be done, and the Ottawa framework. Information will be collected from Objectives 1 and 2. The literature review will assist with the completion of the project plan because it will enable the modification of the present materials provided by the AFHC to the AFHs using the information gathered from the first and second objectives, as well as the data from the literature review. The findings of the data analysis and literature review will be presented to the AFHC Executive Director and staff to help them modify their materials for AFHs. This task will be completed, by February, 2019.

Facilitators and Barriers

There are a number of facilitators who have the potential of aiding the project. The Executive Director and the Education Specialist of the AFHC are expected to be key facilitators. The AFHC is interested in reducing the number of citations among AFHs, and the Education Specialist is in charge of the delivery of training and other resources. Both the Executive Director and the Education Specialist will be asked to provide input for the development of the questionnaires and to facilitate data collection from the intended questionnaire and interview respondents. You also propose to interview nurse delegators and pharmacists.The XY health sciences librarians are also resources to help conduct literature review for Objective 3.

There are many potential barriers that may limit achievement of objectives for this project. Regarding objective 1, the DSHS regional Administrator for Region 2, the pharmacists, and the nurse delegators may have a heavy workload, limiting the amount of time that they can assign to this project. For objective 2, the questionnaires will be distributed by email to the AFHs providers. The AFHs providers may have limited survey response, lack of access to AFHC website due to membership fees or lack of computer skills, language barriers, and lack of reading or writing skills. The AFHs providers may also lack understanding about the project, lack authority to change residents' care, or lack knowledge regarding the management of PRN medications. A negative attitude towards change, and a difference in expectations by the respondents may also be influential factors. Another barrier may be resistance to change. Employees may want to stick to traditional workflows and oppose a recommended change.
