General Characteristics
The facility is a non-profit corporation that participates in Medicare and Medicaid. Nonetheless, it has been established that the facility has automatic sprinkler systems in all the necessary areas by CMS guidelines (Medicare.gov, 2018). Also, the facility has a resident council, which oversees communication between the staff members. However, the facility does not offer multiple housing options and levels of care. The institution provides services to elderly individuals who are chronically ill, debilitated or disabled from a hospital stay or a disease.
Recent Inspections and Issues Identified
The current inspection was conducted on 28th November 2017, and there were no deficiencies; the deficiencies covered areas like resident assessment and care planning, nursing and physician services, resident rights, nutrition and diet, pharmacy services, administration, and environmental (Medicare.gov, 2018). More so, the facility has been rated above average by the Medicare and Medicaid; the institution has a five-star rating, meaning there are very few health risks. Also, in the health inspection report, no single person was affected by any instance of non-compliance.
Staffing levels Compared to State and Federal Averages
The staffing level at the facility takes into account the registered nurses, vocational nurses or licensed practical nurses, physical therapists, and nurse aides. The institution receives about 330 residents per day compared to 167 of New York average and 86 at the national level (Medicare.gov, 2018). However, the total number of licensed nurse staff hours per resident per day is 1.05 hours compared to 1.32 hours of New York average and 1.34 hours of the national average (Medicare.gov, 2018).
On the other hand, the registered nurses' hours per resident per day is 39 minutes compared to 42 minutes of the New York average and 41 minutes on National average (Medicare.gov, 2018). Additionally, the licensed practitioner nurses' hours per resident per day is 26 minutes compared to 49 minutes at the state level and 53 minutes at the federal level (Medicare.gov, 2018). The physical therapist staff hours per resident per day is 4 minutes compared to 8 minutes and 7 minutes at state and federal level respectively (Medicare.gov, 2018). The nurse aide hours per resident per day at the facility is 2.25 hours compared to 2.15 hours and 2.20 hours averages at state and federal levels (Medicare.gov, 2018). Lastly, the registered nurses' hours per resident per day is 39 minutes compared to 42 minutes and 41 minutes at state and federal levels respectively (Medicare.gov, 2018).
Quality Indicators Compared to State and Federal Averages
The quality indicators are based on the excellence of resident care, where the care depends upon a selected set of clinical data measures. However, the residents are divided into two categories, i.e., short-stay residents and long-stay residents. The overall rating for quality indicators is much above the average, for instance, the percentage of short-stay residents who excelled in their ability to move around on their own was 63.8 compared to an average of 69.4% at the state level and 68.0% at the federal level (Medicare.gov, 2018). Also, the rate of triumphant return to home and community for short-stay residents was 61% compared to 51.4% at the state level and 54.0% at the federal level; a higher percentage indicates better quality outcome (Medicare.gov, 2018).
Likewise, the percentage of long-stay residents who report moderate to severe pain at the facility was 1.7% compared to 3.4% and 5.7% average at both the state and federal level respectively, where lower percentages are better (Medicare.gov, 2018). Lastly, the rate of long-stay residents who were physically restrained at the facility was 0.0% compared to 0.5 % at the state level and 0.3% at the federal level (Medicare.gov, 2018).
Five-Star Rating
The higher the number of stars of any activity at a facility, the higher the rank. More so, many stars indicate better quality of care, for instance, the facility has an excellent rating as a result of its service delivery being above the average.
Conclusion
As a nursing home administrator, I would recommend the staff to maintain their mode of service delivery because any medical facility aims to attain a five-star rating, and in this case, most of the rating at the Cobble Hill Health Center is above the state and federal averages. However, to maintain optimal care for elderly residents, I would ensure that I purchase the right medical equipment as well as train the medical staff as opposed to hiring new medical professionals; this would save on cost. Also, I would ensure there is proper maintenance of patient records; this would promote efficient follow-up and discharge of patients. Nevertheless, I would emphasize on the issue of operational policies during board meetings; this would ensure adherence to federal standards.
