As noted by the patient, the pain was felt at the "right upper quadrant and is 'boring' into the back." It is also imperative to question the patient on the kind of food consumed considering that some of the fatty meals lead to the symptoms of Cholecystitis. The fact that the patient is experiencing some fever together with abdominal pain indicates the possibility of cholelithiasis which refers to the gallstones (Mayo Clinic, 2018).

Additionally, some test will be conducted to substantiate whether the patient was suffering from cholelithiasis. Some of the tests to be undertaken include a hepatobiliary iminodiacetic acid (HIDA) scan, a computerized tomography (CT) scan, an endoscopic retrograde cholangiopancreatography (ERCP) and a magnetic resonance imaging (MRI) (Mayo Clinic, 2018).

What treatments or interventions would be the first line of protocol for your diagnosis?

Based on the patient's condition, I will first prescribe for antibiotics. Statin (such as simvastatin, pravastatin, lovastatin, and fluvastatin) monotherapy or with the combination of ursodeoxycholic acid (UDCA) reduces the cholesterol saturation, the gallstone formation and further absorbing the pre-existing stones (Powell 2016).

Second, I will the recommended the patient to take foods with low fat such as red and oil meat, and dairy to enhance cardiovascular health. Foods with low fat include potatoes, vegetables and legumes (for example kidney beans and lentils), rice, paste and steamed or grilled fish or skinless chicken and soy drinks (Mayo Clinic, 2018).

Lastly, if the patient's condition is severe, surgery will be the last option. The two types of surgery to be conducted will include Cholecystectomy- removal of the gallbladder through a surgical process, and Laparoscopic surgery- the use of surgical video camera and narrow tubes passed through small cuts on the remove and repair cuts (Mayo Clinic, 2018).
