According to ( Rodriquez, 2015), tethered patient health record ( PHR) is a significant innovation since it allow patients to view health data such as immunization history, lab results, due dates for a specific screening, medications, discharge summaries, allergies, among other health data, which is all secured through a portal. The tethered PHR also guides patients to access desired medical data from their providers as it enables the users to complete data forms online, requests prescription refills, communicate with the provider, and request scheduled medical appointments. The tethered PHR offers various advantages that include patient-provider communication via protected messaging capabilities and access care via online appointment scheduling

According to (Stombaugh et al., 2015)), electronic health record (EHR) is the initial step to transformed health care. The reason being is that it increases the standard of health care by enhancing all aspects of patient care, such as efficiency, safety, equity, patient-centeredness, education, and communication. Additionally, electronic health records lead to improved incorporation of health care services and promoting preventative treatment strategies by improving efficiencies and lower health care costs. Henceforth, the EHR system promotes healthier lifestyles for the whole population by encouraging better nutrition, physical activity, avoidance of behavioral risk, and broader use of preventative care.

According to (Adler-Milstein 2017), there is a need for a well-managed IT infrastructure incorporated with mobile technology. The integration will assist in smoothening patients' pathway and encouraging patient engagement. The author believes that connected care subsequently leads to enhanced treatment of chronic diseases such as high blood pressure and diabetes. The author gives an example of permanent remote monitoring and evaluation of patients through the use of the mobile application will enable more precise intervention, encourage them to alter their lifestyles for the sake of better results, and guide them to comply with prescribed medication consistently.

According to (Bates et al., 2014), personal health records (PHRs) and patient portals have been significant in their role for improving and increasing patient engagement techniques. The link between electronic health records (EHR) and PHR can exchange health data electronically that guides the patient in getting adequate quality and safe care. However, among the different type of personal health records (PHRs), the tethered PHR is an online interface connected to an electronic health record (EHR) that guides patients in interacting with their health data records thus help patients understand their treatment process.

According to (Doran et al. 2016), healthcare facilities' main ambition is the deliverance of quality services leading to patient satisfaction and wellbeing. However, to achieve such an objective, there is a need for collaborative interdisciplinary team members. Physicians and nurses need frequent updates on improvised treatment methods by taking part in evidence-based practices. The afro mentioned elements are critical in avoiding medical failures, falls, and especially reducing cases of patient readmissions. Therefore, there is a need for evidence-based practices that will equip and enlighten interdisciplinary team members such as nurses and physicians with the knowledge to deal with patients suffering from heart failures. It would be ideal for reducing the increasing cases of readmission that are associated with the hospital.

According to (Michelson et al. 2016), there is a need for health care centers to support optimal treatment planning. The author mentions that typically diagnostic procedures precede treatment decisions, and in that regard, there are various possible treatment options. Henceforth, selecting the appropriate strategy for a particular patient can have a substantial impact on the outcome. The continuous use of clinical pathways, and enhanced by evidence-based practices, can guide motivate health care practitioners to implement treatment approaches proven to yield productive outcomes at a lower cost. Additionally, the author ascertains that the increasing use of electronic health records incorporated with a seamless flow of data between specialists and physicians- and between technology and science can play a fundamental role in ensuring that practitioners are aware of patients' comorbidities. They can exercise caution when formulating medical treatments that would be perfect choices for more conventional patients. Consequently, diagnostic imaging, robust monitoring, and laboratory equipment help support the ongoing, objective assessment of treatment success, which includes entailing intraoperative guidance. In that regard, clinical supervision and evaluation will be significant in optimizing patient results and maximize cost efficiencies.

According to (Revkin 2015), health care payment and delivery models can be formulated to drive quality of care such improving care coordination, establishing primary care as the platform of patient, enhance access to appropriate care and decreasing reliance on specialty services. However, for health care organizations to manage these benefits there is desire for the use of health information technologies that will produce increased quality care at a decreased costs. Advanced technologies ensure there is a limited challenge in aspects such as medical errors, patient's scores, and falls that may jeopardize the wellbeing of the patients. Innovative technologies such as electronic health records are good examples of health information technologies that enhance the quality care of patients. For instance, the use of evidence-based sets of medical orders is a desired change for the treatment of patients suffering from congestive heart failure. The approach leads to enhanced efficiency, and convenience in hospitals leading to health outcomes.
