Introduction
For the people of North Taita to enjoy optimal health levels as individuals and as a population, they should receive high-quality nursing health care in the hospitals. The nursing health care in Matter Health Care Hospital has set out objectives that help them in achieving the optimal health levels. The main objectives include; Explicit purpose to continually reduce the burden of illness. The second objective is as a nurse to ensure the reduction of the weight of injury and disability; lastly, the nursing unit should struggle to improve the health and functioning of the people of North Taita locality and beyond. In so trying to achieve the objectives as mentioned earlier, the Matter Health Care services face different challenges in accessing proper health care. In this paper, we assess the type of health care workflow that matter hospital has applied in the nursing department to their advantage. The analysis identifies the workflow system that has contributed to the overall objectives being achieved.
In the same way, the paper proposes workflow methods that can help the hospital nursing department contribute to the overall objectives of the hospital. In so identifying the improvement methods of workflow, the paper also identifies the possible hindrance that the nursing department is facing preventing the department from achieving the goals that they have set out. Since the nursing department is a huge one, the paper focuses on clinical preventive services offered in the hospital wards by the nurses.
Situational Analysis
Clinical preventive services are medical procedures conducted by the nurses in the wards with a focus on preventing further infection and promoting health care services to the individuals. Clinical health care done inwards is supposed to intervene. Intervention methods are those methods that target to respond to patient symptoms and complain. Such nursing clinical care involve administering drugs, counseling the patient with intention of making sure they change their mental behavior, advising the family on the best medicine to take during the ailing, taking progress record of the patients, registering and administering new patients in the hospital and administering all the interventions that respond to the patient complaints (McGrath, Taenzer, Karon, & Blike, 2016). The clinical practices also involve cleaning up the rooms and making the patient bed clear to admit more inpatients. The provision of proper health care for inpatient contributes about half of the deaths in the hospitals, making it essential to address the issue of health care systems.
Coverage of clinical intervention preventive service has increased in the hospital for the past year. About more than 50% of patients with different forms of insurance receive the best intervention care for the longest time in the hospital. The once with insurances have received the best treatment care, including physical examinations (McGrath, Taenzer, Karon, & Blike, 2016). The people who do not have insurance are submitted to the same treatment, only that they are limited to the way they can access medical intervention. They are expected to pay for the services they receive before the intervention plan is administered to them. For extreme methods of intervention, the nurse's care plan through financial incentives provided by the incentive coverage. The nurses also use data-driven incentives performance measurement strategies to improve the physicians' treatment delivery to the patient. In patients with more care are given an extreme treatment plan by being assigned a nurse to take good care of them.
Even though the trends in the hospital include a proper preventive measure with the goodwill to provide the best intervention plan, some factors arise, hindering the objectivity of the nursing care plan from being achieved. Apart from the challenges from the designed insurance plans, challenges from factors like disparity in the health care intervention plans, caring for chronic conditions, capacity to serve the population, and shortages of health care professionals. The workflow system of the Matter hospital is designed in such a way that the quality of care should not differ because of characteristics such as race, age, ethnicity, income, and education. However, income and the level of education is the one that receives the highest amount of discrimination. For instance, the nursing intervention plan for inpatient bed space is only given to those that have a proper private or government insurance plan. Such a workflow is very discriminative for people with low income or those with disabilities. Evidence from the hospital also shows that minority groups who are not whites rarely receive bed spaces even in the care unit when they are in a condition that needs it. The findings of non-white Taitaers not receiving the quality of care afforded their white counterparts are consistent across all the wards and the entire nursing department of Matter hospital. Reduced health care quality is an independent health care factor that hinders the achievement of the set-out objectives by the health care facility.
The current population of nurses in the health care facility is about a quarter of the inpatient population. The population disparity limits the possibility of the nurses operating with 100% effectiveness. Cases of nurse's fatigue are common in different inpatient wards limiting inpatient effective, caring plans.
The last issue of concern limiting the workflow of nurses is the presence of collaborative care for chronic illnesses. Collaborative care is something the hospital takes very seriously. The nurses are required to partner with the physicians to achieve the best intervention care they can meet. The collaborative care plan interferes with the ethics of the nurses. Nurses are faced with individual dilemmas on the decisions they can make regarding care. Some nurses can't decide for their patients, even if they know what will heal the patient even better. All this returns to the health care system, not meeting the challenges of providing appropriate, cost-effective care for the ones that are chronically ill. The collaborative care protocol system limits the nurses from achieving the objectives of the matter health care facility.
Workflow Requirements
A new workflow system is required in Matter Health care facility. The workflow health care system should help the hospital Nursing department solve the issues that hinder them from achieving 100% effective nursing care. The workflow system should include a health care information technology system inpatient clinical practice. The introduction of IT in the inpatient department might interfere with little protocol workflows, but it will be essential in ensuring success in the inpatient nursing care department.
In terms of the protocol, the nurses' inpatient department doesn't require the nurse to report to the physician. The two should conduct collaborative work so that they both don't end up in ethical dilemma situations. Some nurses have critical care systems that can help the patient, but they are limited to the protocol system that demands them to report to the physician. At times the physicians do not even have the best treatment plan care methods. The protocol method limits the quality of patient care that the nurse should provide. The new process should be one that is slightly more interactive between the two experts. A technological information system could help salvage the situation.
Any health organization requires a proper information system that provides adequate communication between the professionals and the nurses. The communication system also needs to be improved since it relies on large amounts of high-quality information. Communication transfer is complicated because caring for one patient involves many providers, with each of them requiring proper information sources. An error in communication can eminently lead to poor delivery of inpatient services. Verbal order communication is also complicated for nurses to follow through. For nurses, even the slightest of the informal meetings and verbal orders should count. It is to this extent that communication systems through technological information systems should be improved in the health care facility.
Workflow Alternatives
Two alternative workflows can help deal with protocol, workflows, and communication hindrances at the inpatient nursing care unit of the matter hospital.
The first workflow system that can address the problems is an electronic portal. An electronic portal will help the nurse serve each of the patients equally (Yoo et al., 2015). An electronic portal compares and puts the patient names under care in the entrance making it difficult for the inpatients to be discriminated based on race or gender or any other factors. The electronic portal also creates an interface. The interface can be used to communicate between the nurses and the physician (Yoo et al., 2015). The interface solves the issue of protocol. However, the electronic portal doesn't solve all the workflow inpatient systems. The portal limits the nurse's verbal interactions with each other. The oral communication is so important since they help detect even the slightest of the mistakes. Such limitation makes the less portal less adequate to mitigate and adequately address the communication needs of the care team. The most common class of problems with the system is that it also degrades.
A second important system that can be used is health information technology. The health information technology has been proven to improve efficiency and organizational workflow. In terms of protocol, important communication in the team is created and stored. Nurses faced with ethical dilemmas within the organization can easily reconcile information from the system and use it to make a decision. The system quickly solves issues of ethical dilemmas of nurses (Sikka, Morath, & Leape, 2015). Secondly, health information technology improves inpatient care delivery through a system where the patient's register. After registration, they are at will to adequately consider the depth to which they will be treated when they access services in the hospital. It means the system solves the problem of discrimination experienced by Matter Hospital.
The health information system solves the issue of communication by analyzing the communication workflow ahead of time. Essential communications are made to the nurses through the hospital portal. In the interest of quality care, the nurses can get critical conversations with enough time, enabling them to act on them. The system's only limitation is that it is not applied in nursing practice schools. However, the nurses have to use it with all the technicalities that surround their operation.
Recommendation to the Board
It is logical to ask the board to implement the health information technology system, which will act as a remedy to the problem of protocol, communication, and disparity in the provision of health care in the organization. Most importantly, the system will help solve the workflow error, thereby improving the nurse's delivery of attention to the inpatients in the ward.
It is also recommendable to the board of the hospital that before they enroll in the health care technological information system, they should conduct in-practice training to the nurses (Mensah et al., 2015) An in-practice training to the nurses will go a long way in ensuring that the nurses deliver the quality care without the new system hindering or changing the workflow.
