Patient-Centered Medical Home would be effective for healthcare units in delivering quality service to the people or patients. Medical/ health homes have been provided in most areas, and they help the patients with primary healthcare practice and access. Again, this liberation care model helps the patient to receive effective treatment that is coordinated through primary care physicians. This helps the healthcare provider's to concentrate on chronic and preventive care management solutions. These solutions are aimed at reducing the dependence on emergency and specialist care. Health care transitions are ideal activities that have been based on an extensive care plan and the provision of well-trained health providers who are up to date on the current information about the patient's treatment preferences, objectives, and clinical or health status (Blanchard et al., 2017). Multiple provisions are being developed on affordable healthcare act to improve healthcare delivery. Most of which are trying to demonstrate the reducing cost, although it is still inadequate. The overall quality and coordination of the patients are also becoming one of the more visible approaches aimed at the provision of better healthcare. This incorporates all the stakeholders and the society, as well as the medical practitioners (Marsilio, Torbica & Villa 2017).
The interpersonal and interdisciplinary professional teams can then decide on the best practices to utilize in the healthcare delivery. Consequently, effective transitional healthcare can prevent medical errors, support consumer's preferences, identify issues at early stages for intervention, avert unnecessary readmissions and hospitalization, and reduce duplication of services (Blanchard et al., 2017). Therefore, effective healthcare transition can improve the quality of care while utilizing the available resources efficiently. Ultimately, assuring to a safe, integrated, high-quality, and well-coordinated healthcare delivery system is tantamount to reducing wasteful spending, improving healthcare outcomes, and reversing the spiraling cost curve (Marsilio, Torbica & Villa 2017). The emerging models of MIH help to leverage the EMS systems to adequately address the significant issues of unplanned episodes of care, longitudinal care, and care transitions by utilizing existing resources more efficiently and enabling information and data sharing among healthcare providers and the health systems.
