Introduction
According to the Nurses Association in America, nursing informatics is the expertise which joins nursing science together with several analytical and information sciences. The sciences are used in identification, definition, management, and communication of the DIMW continuum. In today's changing and advanced technologies, informatics competencies have seemingly turned out to be important to the informatics nurse, health care professionals, and regular staff nurses (Topaz, 2013). The parties mentioned above ought to learn how to use the modern technologies in their practices. Among the most important nursing informatics competencies which are needed of every nurse is the knowledge and ability to use the database in searching and retrieving the required information to be used in their practice. The paper demonstrates how one can research a clinical question, information extraction and organization of the data obtained, the application of the DIKW concept in research and the use of knowledge, together with wisdom gotten in the next practice.
Clinical Question
While conducting research, the initial step in the development of a clinical question that the study needs to answer. The clinical question, in this case, aims at identifying patient's risks due to poor results during the patient's transmission from hospice care to home care. The safety of a patient is of undue prominence hence the reason why it has gained more focus in the recent years. Now and then, nurses are engaged in the study and management of various transition types which include managerial development, and immigration transition. Over the years, the transitions theory is seen to play a paramount role in nursing research, practice, and knowledge. Individual conditions influence changes from hospice care to home care. That means that high family support level may lead to a patient being transited from hospital to home, especially for the old aged patent.
To get to the answer to the clinical research question, I made use of EBSCO databases under Medicine and CINAHL databases. The research was conducted by the search of the keywords "hospice care to home care, patient's risk and transition" since the database would limit the search to articles that contained a combination of the keywords. Additionally, another limiter used to restrict the results is specifying to view research journals that were peer-reviewed and those whose research was completed in a range of ten to fifteen years prior. The above limitation was done to get the most recent and applicable research data.
After narrowing down the journals and articles using limiters, each article's title is read through to determine if it meets the research question's needs. Most titles which are available within the database should be given priority. After which the article link is clicked to bring a view of its abstract with the aim of identifying if the articles contain relevant information about the research topic.
Relevant Information Extraction and Organization
When answering a research question, it is important to extract and organize the significant information. The first step is to scan followed by the reading of the chosen material while highlighting important information. After reading, a table, to help in the organization of the information garnered is made. The table is meant to get information more straightforwardly from the research materials hence can have categorized with subheadings such as Author, title, aim, methodology, results, and conclusion. After the typing and organizing of that data, it is easier to use the relevant information for personal knowledge.
Knowledge of Wisdom Progression. The new technology advancements use supports the nurses in their patient care for a patient's instant change and new service development. The therapeutic services specialization, nursing treatment, and consideration are added up by the technological advancements. The above ought to intensify tests of the ways of handling predicaments and avoid damage during the transitional period. The use of advanced instruments intentionally is among the core methods needed to deal with the social challenges of insurance that may be anticipated. Commitments for several ranges are incorporated by wellbeing informatics. The few ranges include informatics from the field of medicine, nursing, clinical and dental (Moen, & Knudsen, 2013). The health and biomedical informatics are seen to be care providers to customized treatment and driven health systems that are capable of controlling unending conditions. Additionally, the informatics add their diligent efforts to early intercession, as hindrance actions move to meet the required needs for a mature setup.

Over time, the efforts portray the different subdomains which are found in the health informatics interdisciplinary field. They who seek the profession of health informatics are often highly educated to join the area with full skill mastery and knowledge. From there, it is the role of the skilled informatics to use the acquired knowledge to use the technological advancements to the benefits of the medical institutions, the patients and the medical field at large (Matney, Avant, & Staggers, 2016). This way, they will be able to transform the knowledge gained into wisdom.
