Healthcare nursing, priorities and roles are one of the diversifying sectors in the leadership aspect as noted by several studies. In the finding, the nursing leadership aspect has shifted to providing higher-quality care at a lower cost especially by hospitals. Upgrading the healthcare has attributed to the direction where each in the healthcare will be accountable for their health beginning with the patient to the physicians. According to recent research on the growth healthcare, the rate of transformation related to the healthcare will mean that the healthcare system will shift the objective of the healthcare from an illness system to a preventative and wellness system since it will help in managing patients` illness. Similarly, the healthcare providers are deemed to change their way conducting their duty just like the transformed healthcare systems. It is widely noted that various hospitals have received enough funding from the patient's fee which may be used to run the health care activities that are need fully required by the patients. Moreover, when the patients receive poor attendances at the Healthcare, the hospital's reimbursement is affected. Hence, the future for strong healthcare institutions will be achieved through operating at high-quality, effective care by generating a culture of a valued base system.
According to various scholars, different nursing priorities are perioperative services in this subject. Cultivating nursing attitude is a pertinent issue since it enables one to accept changes as they appear. In this regard, a nursing leader must, therefore, cultivate a nursing attitude that will provide the direction of the organization of implementing changes as well as the reactions of various nursing units. Taylor (2014), suggested that for one to possess success in the implementing changes in the perioperative environment, one must possess a positive attitude and similar perception as the organization demands of the hospital. In addition, a nursing leader must have an understanding of how to handle various crisis. Other understandings required of the nursing leader include adjusting roles, shifting, and habits of nurses in order to make the perioperative area successful. According to Taylor, a successful nursing leader must acknowledge that the department has a responsibility in ensuring the department remain successful. As a result, the leader will achieve an excellent performance in a perioperative department. According to Anderson (2011), a new act on the affordable healthcare have led to the reimbursement of new regulations in various hospitals. This initiative has contributed to the paying of the hospital regarding value but not volume. One of the relevant management skill among the nursing management and leaders is to accomplish offering an improved quality standard of care as well as providing safe and efficient care to the patient. This paper will endeavor at establishing one of the most remarkable aspects of nursing leadership which involve increasing the patients' satisfaction by proper enhancement of the nursing performance and position in healthcare.
This presentation gives account of leadership in practice in an 18 bedded acute inpatient ward and relevant critiques and controversies regarding leadership in Nursing. In addition, the presentation gives account on the situational leadership theory where I was exposed to my clinical mentor took control and made decisions about staffing to ensure that the seriously ill patients are provided with the best care. The Gibbs's reflective cycle has helped me in making sense of the condition and to put into the right perspective. I have recognized how to put the learning experience to affirmative use in my future practice as caring nursing professional.
SLIDE 2 INSTANCE OF LEADERSHIP
During the shift, I was allocated to work alongside the senior nurse who was also my mentor. A staff member had an emergency at home and the senior nurse had to iintervene. The senior nurse was left with no choice and took the decision to go to another ward to request for assistance to cover the absent colleague. When reaching the ward, which appeared to be overstaffed, he was using his leadership skills to procure a member of staff to come and assist with care delivery, as some patients on the ward were classed as being at risk. This leadership style did not prove fruitful as the staff were reluctant to leave the patients they were assigned even though they were not in a critical condition. At this point, he changed his leadership style into authoritarian in order to ensure the staff were working with the most at risk patients. This incident was chosen because it highlighted to me an example of the situational leadership model as he dealt with an evolving incident which required him to change tactics
SLIDE 3 WHAT IS LEADERSHIP IN NURSING?
Leadership can be defined as the ability to give direction or guidance to a specific group of people. It also means respecting and believing in human dignity. In nursing leadership means listening, being attentive, encouraging, upholding a reward tradition and inspiring. A good nurse leader understands that leading is a comprehensive management that is consistently focusing on good quality. This means that a good leader will make sure that every nurse knows that very high-quality care for patients is key. Leadership in nursing should be consumer orientated that is understanding a patient completely and not just in terms of the illness.
A good nurse leader should be looked at and admired by others as a good role model (Seitovirta, 2017). Being democratic is another value of a good nurse leader. This kind of leader ensures that everyone including the junior staff is included in any decision. When all the staff feels valued then they tend to give their best. There are many values a good nurse leader should have and the most common ones are the 6 Cs. One of them is compassion, a good nurse leader should be compassionate. This means that a good leader should be able to care for everyone including the junior staff.
Courageous is another value of a good nurse leader. In nursing being faced with difficult situations is the order of the day. Therefore, a good nurse leader should have the courage to do what is right at all times. A good nurse leader should also be able to communicate effectively. Communication is important in all aspects of nursing from communication to patients to the rest of the nurses. Another value that a nurse leader should have is competency. A competent leader has the knowledge and skills required to provide the required care. This means that a good leader does not stop learning and developing new skills.
Care is also a very important value in nurse leaders. Being able to give high quality care is not always easy but every nurse has to work towards it. High quality care has to be a priority for every nurse. Lastly a good nurse leader is committed. Due to the many pressures that come with the nursing profession, it is important that every nurse realizes the reason why they joined the profession and be committed to it. It is important to note that good nursing leadership is all about empowering others. People should feel comfortable talking and following in the foot steps of a good leader.
The Wanles report on leadership in nursing says that good leadership depends on having the right tools and investments in place to help leaders pursue their leadership responsibilities. It goes on to say that effective leadership in nursing benefits from leaders having the freedom to do what is right. That is a working culture that empowers leaders and gives them the authority identify and challenge bad practices. This freedom allows leaders to discharge their duties effectively without the fear of being challenged.
A research done by Scully (2013) showed that personal behaviors and characteristics of good nurse leaders have led to healthy relationships in the work place. Nursing is a challenging and dynamic profession and there is need for leaders to guide others. This research says that it is challenging to find nurse leaders but it has to be done due to the importance of having leaders in the nursing profession. It is very important to have leaders who have the knowledge and skills to lead others (Scully, 2013).
SLIDE 4 SITUATIONAL THEORY LEADERSHIP
Situational leadership can be described as a leadership style that is dependent on the situation at hand. This kind of leadership is adaptive to the current situation. This approach's main aim is to bring together two elements that is the follower's maturity and the leadership style of the leader. The situational theory leadership was developed by Hersey and Blanchard in 1969. They both later wrote books on the same. Blanchard first book was called 'THE ONE MINUTE MANAGER' and it was produced in 1982. Hersey also wrote a book later called 'THE SITUATIONAL LEADER' and it was produced in 1985. Both of this books have been used to explain the situational theory leadership style in the world today.
This theory says that the effectiveness of a leader is related to the leader's behaviors in different situations. The reason why this theory was developed was to improve productivity. The main responsibility of any leader or manager is to raise or increase productivity of the company or organization. By using the situational theory leadership style, the morale of employees in a given organization can be build leading to better productivity.
There are four different styles in situational theory leadership. These are mainly related to supportive behavior and directive behavior. The meaning of supportive behavior is giving or providing support and guidance. Directive behavior can be described as simply as giving an order. In today's world leaders are no longer bosses and they can not only lead on the basis of their position. Leaders have got to have qualities that others can emulate from. The four leadership styles that Hersey and Blanchard used are:
- Telling - in this kind of leadership the leader tells or demonstrates low supportive behavior and high directive behavior. This means that the leader tells people what and how to do something.
- Selling - here the leader tells and demonstrates both high supportive behavior and high directive behavior. This involves back and forth of ideas between the followers and the leader.
- Participating - this approach allows the leader to give high supportive behavior and low directive behavior. The followers here are given the role of making decisions and coming up with new ideas.
- Delegating - in this leadership style, the leader demonstrates low supportive behavior and low directive behavior. Here the followers are responsible for their actions since they are making most decisions.
A follower's maturity depends on the ability of the follower to be able to perform tasks either physically and psychologically. This is also known as tasks maturity and psychological maturity. A leader's duty is to determine the follower's tasks maturity and psychological maturity so as to know what kind of leadership style is best for a specific follower. It is true to say that the more mature a follower is, the less directive behavior will be used on them.
A research that was carried out in three hospitals in South of Brazil showed that coaching style was dominant there. There was need to introduce a directive behavior kind of leadership to improve the performance of the nurses. The findings showed that the nurses were not empowered in regards to their working process which is an important aspect of the situational theory leadership. This shows that the leaders do not inspire their followers which would mean negative and poor results (Cristina, 2016).
