Change is very important towards achieving the success of every business. The Kotter's 8 step change model includes 8 steps that the management can implement to successfully implement change in the organization. Falls is a major health care concern among the geriatric psychiatric population. Falls usually occur in all hospital units but the falls tend to be more frequent in the psychiatric units. This essays analyzes how step 1 and 2 of the Kotter's change model can be utilized to bring change that will help in reducing the falls among the geriatric psychiatric acute inpatient population.

The change process will involve the management fitting pressure sensors on the bedside of the geriatric psychiatric patients and as well as use of a radio-paging alert mode which will help in the creation of awareness to the nurses and doctors in the hospital anytime a patient wants to rise from their bed. For the process to be effectively implemented and to enable the change to occur the entire hospital management should support the process.

While implementing the change process the management should consider the stage 1 of Kotter's change model by evaluating why maintain the status quo is unacceptable. There are various potential threats that are associated with the falls among the geriatric psychiatric acute inpatient population. The falls results in a patient staying for a longer period in the hospital. It also increases the hospital charges which the patient is required to pay. The status quo is also unacceptable because the fall of a psychiatric acute inpatient could result in death if there is no one near to help the patient and identify any damage that could have occurred.

The feedback from the patients as well as the internal and external constituents that leads me to make the proposed change is that after the fall the patients suffer from severe headaches and internal bleeding as well and therefore their discharge time from the hospital is extended. Most of the patients are also not in a position to pay the hospital bills which tend to be high due to the extra changes that the patient is required to incur.

The members of my guiding coalition include senior administrators in the hospital, clinicians and the Chief executive officer. I chose these people to be involved in my change coalition because of the knowledge they have which will enhance a successful implementation of the change in the process as well as their position of power. These are people who will help me in c convincing the other physicians within the hospital that change needs to happen and the important of the change.

The chief-executive officer has the highest ranking executive in the hospital and therefore plays a significant role in decision making on how the change should be implemented and as well as manage the overall resources and operations that will be required and used to enhance the implementation of the change. The expertise of the clinicians will be important during the implementation of the change as the clinicians have the skills to assess the problem of the patient and hence they will use their expertise to kelp assess if the pressure sensors will have an impact on the health of the patients. The credibility of the members of the guiding coalition will be enhanced by ensuring that the group is seen and respected by everyone so that the pronouncement of the group will be taken seriously by other people .Their credibility is enhanced by ensuring that they are able to tell the truth regarding the conditions of the geriatric psychiatric acute inpatients who fall since they deal with the patients directly. The senior administrators and the chief executive officer are leaders who have proved that they can drive the change process. They are in a position of empowering the junior workers and other physicians in the hospital to embrace the change with positivity.

For a successful implementation of change by the coalition the members should ensure that they trust each other through communicating openly about their thoughts and ideas regarding the change. To enhance the mutual understanding of the coalition decisions made should be critically analyzed to asses how effectively the coalition is making clinical decisions that are collective. There should also be deliberative and open discussions to enhance critical self-reflection. For the change to be implemented successfully the guiding coalition should have a culture of respecting one another. This is through helping each other, listening to each other, and encouraging everyone in the coalition to be themselves. The guiding coalition show care by ensuring they evaluate how the change system could affect others negatively, going above and beyond to personally help each other, and also be real and transparent with each other.

The common goal within the guiding coalition regarding the proposed change is to ensure that after the implementation of the change which is fitting the beds of psychiatric patients with pressure sensors the number of falls will be reduced significantly as it will help in alerting the nurses that there is a patient rising from their bed. The goal was to implement the change within the shortest period possible.
