Introduction

The service user and caregiver involvement in the mental health systems are considered the major expectation of policies in some countries. It has thus led to the attraction of interest in the growing research across the world. The service user and caregiver in the mental health system concern to the policy-making, service development and delivery, strategic planning, research, training and education, quality assurance, peer support, and advocacy in the health system. It can take place in the direct care planning, assessment and case management, the health facility and community, and the macro level of the environment (In Jordan, M., & In Middleton, H. (2017). They all have a potential of registering several involvement degrees.

In this regard, the involvement indicates the injunction of democratization of health services, transparency and public accountability which can lead to the accessibility and acceptability of the health care system, enhancement of the relevant involvement of services, and responsiveness of the mental health care services offered by the healthcare practitioners. The service user and caregiver approaches in mental health care are deemed to be a multifaceted construct that has various meanings, processes and levels. The review of the service user involvement in the mental health care describes the context, barriers, type, and facilitators for the participation of the general lessons for the good of the practice. Since the realistic review methodology has not been used in the mental health service involvement in low and middle-income countries, the paper aims at describing how the contextualized approach will get applied for the identification and refinement of the theory-driven and transferable model of involvement of service users as well as the caregivers.

Method

The methodology used in the study goes through a five-step process. The methodology also involves: design and realist view

Owing to the extent of complexity of service user and caregiver involvement, the realist view can be a key methodological tool for the facilitation of the exploration and learning, a collection of the evidence and the transfer of the lessons from the study to different contexts. The five-step processes followed in gathering data for the study are the:

  1. Clarifying scope; In this case, the initial program theories' review questions will get to be identified in relation to the ToC map which is developed through the experimental knowledge analysis for the stakeholders which tends to set out the framework of the method intervention, the outcomes as well as the indicators (Kennedy, G. J. 2012).
  2. Searching for evidence; After the development of candidate program theories and the draft ToC, a diversified purposeful search for the peer-reviewed literature will be performed to seek fitful evidence for the refinement of the candidate theories and evidence (Johannes, C. K., & Zee, H. . 2010). The search methods will focus on the articles of the service user and caregiver involvement in the high income and low-income countries.
  3. Screening and selection of studies; is done by the use of the title, abstract, and the full text. They are read both the exclusion and exclusion criteria used to decide on the relevance of the articles.
  4. Data extraction and quality appraisal; In this case, the screened data will be assessed for the determination of the credibility of the research findings. Also, they are the theoretical assertions for each study undertaken. In quality appraisal, there is the drawing of evidence from a wider range for use in the literature review. The studies are assessed based on its applicability to the theory in practice, alongside the trustworthiness of the methodology. Data extraction is done basing on its relevance to the question at hand.
  5. Analysis and synthesis; is done for the extracted data using realistic logic to question the program. The essence of the collected data is assessed based on the research questions. Its meaning to the research is then found out as well as its implication basing on the methodology used. Its relevance to the matter at hand is also assessed by the simplification of the research findings by the use of graphs and tables.

Results

With the conduction of the research study, there was the resultant outcome that came out. They include the fact that the synthesis of evidence through the realist review is complex relatively neglected and underutilized in the area of service users and caregivers involvement. However, they can strengthen mental health systems, more significantly in the low and medium income countries.

Discussions

The service user and caregiver involvement have given rise to interests as a means to strengthen the mental systems of healthcare across the world. There are potential benefits to service providers, health facilities, and the general health system. The involvement of service users and caregivers in the mental health care system is quite a complicated process and influenced by a seamless interplay of the service provider, health system factors, and health facilities. It thus explains why it is hard to practically implement and develop the models on the best ways to involve the service user and caregivers.

Personal Insightful Critical Analysis

The realist view can provide sound evidence in theory-driven form. There are raised questions regarding the nature of the mechanisms as well as the challenges of making a difference between the mechanisms and the essential context conditions. The consensus to arrive at the key terms and concepts shall be made through discussions to solve the above problem. There is a challenge to reproduce the review in the realist reviews because of the use of a mixture of processes and evidence and as opposed to the systematic reviews in the conventional reviews.
