The primary reasons for the fall of the jet are management challenges. The decision making was one of the core issues that lead to the fall of The Challenger, especially on the eve of its launch. Based on Adams's effort to stop the whole process from being approved, the holding stakeholders could not give him any chance to prove himself. The NASA management body was more concerned about their contracts and the vast income that would come from the launch of The Challenger. The management had only one decision to make; to launch the whole process and secure their names amidst the numerous criticisms and blames from all over the region. The management could not consider the fact that the chilly weather could not allow for the launch to be successful as compared to the prerequisites launches.

Possible Preventive Measures

The first preventive measure that could get taken into consideration is the enactment of the act of collective responsibility. The law of collective responsibility allows for all the holding parties to pronounce their views towards the process. Taking every perspective on a serious note, it would have allowed the corporation to revise the whole process before the launch. NASA Corporation should have considered all the criticism and concerns that were raised towards the start. Adam could have earned more time for the delivery concrete reasons why he felt the whole process was a mess and would endanger the life of the space pilots and the entire team. The management could have taken into consideration the weather condition at hand that leads to the burning of the O-ring with a direct effect on the two seals as argued by Adam. There was a need to reconsider using the O-ring concerning the temperatures experienced during the launch.

The Challenger’s Impractical Launch

Several instances show the whole process of launching The Challenger was impossible. There were experienced delays in the launching hours and dates based on the original tabled framework. Several malfunctions were prompting the whole process from being positioned to action. The first attempt they realized there was a leakage in the first main engine. The same problem was recognized some months later when they found a minute crack imposing the same leaking leakage. The management's desire to secure their multibillion projects other than the considerate value human life at stake was raising many questions. The image presented through their concern was imposing a great danger to the seven astronauts on board.

The Ones Responsible for the Explosion

The blame for the fall or explosion of The Challenger goes to the management and policy maker's team. Despite the numerous claims of impending danger from the then weather focus, the management never considered revising their stand to call off the launch of The Challenger. They created a short time for the critics such as Adam to prove as to why the start was inappropriate. The safety department was not keen on the outcome of the start. There were several pre-warnings from the rocket like the cracks and leakages from the engines that were to be taken care of for a long-term result. The safety team was negligent on the weather patterns and how they relate to the operation of the engine. As explained by Adam, the O-ring would wear out leading to unsealing of the two engines hence explosion. All these measures got subjected to the management who was clumsy to consider. In anecdote, the administration had a single preset mind other than a public concern.
